Identify the elements of SOAP charting for the following scenario: A patient has come to see the doctor complaining of asthma. She says she has been wheezing and coughing for two days. The patient's vital signs are P–102, B⁄P–146⁄100, R–28, T–98.6. Following examination, the physician determines the patient is having an asthma attack.
