On HealthQuest, choose the Wellboard module and click on the "Assess Yourself" icon. You will be able to answer questions in five categories: General Demographics, Family Health History, Personal Health History, Lifestyle Questions and Preventative Health Practices. After answering each question, read the section entitled "Why is this important to me?" to discover why the answers to these questions have an impact on your health. In the Lifestyle Questions category, you will see numerous sections entitled "Why is this important to me?" that contain information about tobacco, alcohol or drugs and related topics such as drinking and driving. As you complete the assessment, pay special attention to the summaries you receive regarding your use of tobacco, alcohol, and drugs.
