Authorized depository  A bank that can accept payroll deposits from its own checking account customers.
Employer identification number (EIN)  A number issued by the federal government that uniquely identifies a business.
Employer's Quarterly Unemployment Tax Report  A report filed with the state that shows an employer's unemployment taxes owed.
Federal depository bank  Bank authorized to accept deposits of amounts payable to the federal government.
Federal Insurance Contributions Act (FICA) taxes  Taxes assessed on both employers and employees; for Social Security and Medicare programs.
Federal Reserve Bank  A bank that can accept payroll deposits from any business.
Federal unemployment taxes (FUTA)  Payroll taxes on employers assessed by the federal government to support its unemployment insurance program.
Form 940  IRS form used to report an employer's federal unemployment taxes (FUTA) on an annual filing basis.
Form 940-EZ  The Employer's Annual Federal Unemployment Tax Return. This shows the amount of FUTA tax the employer owes for the year.
Form 941  IRS form filed to report FICA taxes owed and remitted.
Form 8109  A preprinted Federal Tax Deposit Coupon. It is used when an employer deposits money into a federal depository bank.
Form 8109-B  A Federal Tax Deposit Coupon used by new businesses or when the business does not have a supply of preprinted Forms 8109.
Form SS-4  An Internal Revenue Service form filed by a business in order to receive an employer identification number.
Form W-2  Annual report by an employer to each employee showing the employee's wages subject to FICA and federal income taxes along with amounts withheld.
Form W-3  The Transmittal of Wage and Tax Statements form. This form reports the total wages and tax withholding information for all the employer's employees for the year.
Look-back rule  A rule used to classify business as monthly or semiweekly depositors.
Merit rating  Rating assigned to an employer by a state based on the employer's record of employment.
State unemployment taxes (SUTA)  State payroll taxes on employers to support its unemployment programs.
Workers' compensation insurance  An insurance program that provides benefits to workers who are injured on the job.
